The DP4 menu page toolbox is used exactly like the ordinary Map Editor toolbox as described here. It contains the following tools:

Text Controls are used for adding background or informational text, not associated with menu options.
Option controls are numbered text controls which can be selected and clicked to invoke a menu option.
Hidden Option controls are numbered options in the menu which do not have a visible control for selecting them. The most common hidden option is option 0, selected when the user escapes from a menu page, but you can set up other numbered hidden options if desired. Hidden options are displayed at the top left of the page at edit time, but are invisible at run time.
